This page is divided into three short form sections. Each section may be completed separately and updated later if needed.
Before You Begin
To make the estate planning process as smooth as possible, we recommend gathering the information below before completing the forms. You do not need to have every answer available, but having this information nearby will make completing the forms much easier. If you have questions while completing the forms or are unsure how to answer a particular question, simply leave it blank. We will review your responses together before finalizing your estate planning documents.
Information is used to prepare California estate planning documents based on your written instructions.
Form 1-Family Information & Guardianship
This form gathers information about you, your family, and the people you trust to care for your loved ones.
Before you begin, please have available:
- Your full legal name and contact information.
- Your spouse's or domestic partner's information (if applicable).
- The names and birthdates of your children.
- The names, addresses, and phone numbers of the individuals you would like to serve as guardians for your minor children.
- The names and contact information of anyone you would like to manage your children's inheritance.
- Your preferred age for children to receive their inheritance.
Form 2- Assets & Beneficiaries
This form identifies your assets and who should receive them.
Before you begin, please gather:
- The names of the people or organizations who will inherit your property.
- Any backup beneficiaries you would like to name.
- Property addresses or Assessor's Parcel Numbers (APNs) for any real estate you own.
- Information about jointly owned property.
- Business ownership information, if applicable.
- The names of your banks and financial institutions.
- The last four digits of bank, brokerage, and investment account numbers (full account numbers are not required).
- Information regarding retirement accounts and life insurance policies.
- A list of any valuable personal property you wish to leave to a specific person, such as artwork, jewelry, firearms, collectibles, or family heirlooms.
- Any charitable organizations you would like to include in your estate plan.
Form 3- Legal Appointments and Directives
This form allows you to choose the people who will manage your affairs if you become incapacitated or after your passing.
Before you begin, please have available:
- The names, addresses, and phone numbers of the person you would like to serve as Executor of your Will.
- Your preferred Successor Trustee(s), if creating a trust.
- The individual you wish to appoint as your Financial Power of Attorney and one or more backup choices.
- The individual you wish to appoint as your Health Care Agent and one or more backup choices.
- Your preferences regarding when your Powers of Attorney and Advance Health Care Directive should become effective.
- Any additional instructions you would like included in your estate plan.
